The case of a 54-year-old male with a compression fracture of the T 12 vertebral body and a bilateral anterior column fracture of the acetabulum is presented. The origin of this rare combination of fractures is explained by the action of the patient's own muscle force on bone of very poor quality. The treatment of these lesions is comparable to the treatment of similar fractures in bone of normal quality.
Influence of the level of arterial resection on the replanting and revascularization results in hand surgery: prospective study over 22 months

Abstract The objective of this work was to assess whether the injury mechanisms are responsible for histological arterial lesions. This prospective single-center study included adults with wrist or hand arterial injury. Arterial resection of at least 2 mm from the proximal and distal stumps was performed before the arterial anastomosis. Histological analysis of the arterial stumps was performed. An ultrasound was performed 1 month postoperatively to check arterial patency. A clinical and functional evaluation was done at 1 month postoperative, then every 3 months. Ossification of the Achilles tendon is the result of a traumatic, hypoxic injury of the tendon. The usually asymptomatic ossification has a clinical importance only in case of a new trauma. We report about a patient with an isolated fracture of the ossification without an accompanying rupture of the Achilles tendon. Because of the radiologically "occult" fracture the diagnosis could only be verified by MRI. With a conservative treatment without resection of the ossification we could reach painless recovery.

Total hip replacement is often recommended for the treatment of femoral neck fractures in elderly patients. A less invasive approach with shorter operating time is the replacement of the femoral head only, leaving the acetabular cartilage in situ. Hemiarthroplastic hip replacement using a metallic head prosthesis was not successful in the past, because the majority of patients developed protrusio acetabuli after surgery. In the present study hemiarthroplastic hip replacement using an Al2O3-ceramic head was shown to be a useful method for the treatment of femoral neck fractures in elderly patients. Background and Aims: Michigan Hand Outcomes Questionnaire is a widely used patient-reported outcome measure in hand surgery. The aim of this study was to translate and validate the Michigan Hand Outcomes Questionnaire into Finnish for Finnish patients with hand problems following international standards and guidelines. Material and Methods: The original English Michigan Hand Outcomes Questionnaire was translated into Finnish. Hand injuries after high-pressure injection are a medical emergency. These events occur frequently in workers during industrial cleaning, painting, and lubrication, and may have devastating consequences, leading to eventual amputation and poor functional outcomes. In einer retrospektiven Studie wurden die Begleitverletzungen, Behandlungsmerkmale, Morbiditat, Mortalitat und das radiologische und funktionelle Endergebnis von 222 Patienten mit einer Beckenringverletzung vom Typ B oder vom Typ C nach der Klassifikation von M.Tile eruiert. 52 Patienten wiesen eine "Open-book-Verletzung" auf (Typ B1), 48 Patienten eine laterale Kompressionsverletzung (Typ B2/B3), 122 Patienten wiesen eine C-Verletzung auf. Tuberkulöse Destruktion des Iliosakralgelenkes

In dieser Kasuistik wird der Fall einer tuberkulosen Destruktion des rechten Sakroliakalgelenks eines 56-jahrigen Patienten vorgestellt. Auffallig sind die verspatete Diagnose durch Verwechslung der Beschwerdesymptomatik mit einer Lumbalgie durch Bandscheibenprolaps und die Abszedierung entlang der Psoasmuskulatur und in die umgebende intra- und extrapelvine Muskulatur. Die Therapie ist eine chirurgische, die durch eine einjahrige medikamentose Kombinationstherapie erganzt wird.
Impact of COVID-19 on hand surgery in Italy: A comparison between the Northern and the Southern regions

The aims of this study were to evaluate the impact of the COVID-19 pandemic on emergency and elective hand surgery in four Italian regions that had either a high (Lombardy and Piemonte) or a low (Sicilia and Puglia) COVID-19 case load to discuss problems and to elaborate strategies to improve treatment pathways. A panel of hand surgeons from these different regions compared and discussed data from the centers they work in. The COVID-19 pandemic had an enormous impact on both elective and emergency surgery in Italy, not only in highly affected regions but also – and paradoxically even at a higher extent – in regions with a low COVID-19 case load.
